In the service of life....
Palliative care....
is for those people and families who are facing a life-threatening illness which has reached the point where a cure is no longer possible.
The goal of palliative care is comfort and dignity of the person - to achieve the best quality of life possible based on his or her unique physical, psychosocial, cultural, emotional and spiritual values, and needs.
Palliative care is a commitment to whole-person care.
Palliative care.....
.... is available in the home.
Some people wish to die at home. If this is your choice, please discuss this with your health care providers. Every effort will be made to provide care and support to you and your family.
.... is available in the hospital.
If you wish to die in a health care centre or if your care needs can no longer be met in your home admission to a hospital is available. Care and support for you and your family will be provided.
.... is available in personal care homes.
Residents are cared for in the personal care home unless their care needs can no longer be met. Every effort will be made to provide care and support to the resident and their family.
Volunteers
Volunteers are an important part of the Palliative Care Program. Trained volunteers provide compassionate and confidential care for you and your family at home, in a hospital or in a personal care home. Their services may include home visits, social activities, transportation, providing time off for caregivers, errands such as shopping, and support in time of grief.
A common hope
At some time, in some way, we must all face the end of life. And most of us share in a common hope - that when death comes to us or to a loved one, it will be peaceful and free of pain. We hope to be surrounded by those we love, feeling safe, comfortable and cared for. The Palliative Care Program has been designed to make these hopes reality.
Assiniboine Palliative Care Program
The program champions the development of palliative care for residents of the Assiniboine Region through education, information, advocacy and support to service providers.
